Consent & Liability Release
In the handling of your personal DNA profile results, it is the intent of the Winslow Family Y-DNA Project Coordinator to ensure to the best of his ability the strict confidentiality of the specific identity of the participant in conjunction with the participant's personal Y-Chromosome DNA results. However, the individual DNA data for each participant will be posted on the Winslow Genealogy web site, and possibly other future literary expressions (books, etc.) in the manner prescribed below.
Unfortunately we live in a time when an individual, who chooses in good faith to expend the significant personal resources necessary to coordinate such a project, also potentially exposes himself to unwarranted liability. Thus, based upon advice and counsel, I have included the following waiver of liability form that will be required to be executed by all Winslow Family Y-DNA Project participants before the Project Coordinator will integrate your DNA Profile data and post it at the public Winslow Genealogy web site. The form is shown below for your review and approval.

This Release and Waiver is executed in the interest of permitting the Winslow Family Y-DNA Project to go forward in the genetic genealogical testing of all participants and of permitting Stephen J. Winslow, Sr. to serve the Winslow Family Y-DNA Project as Project Coordinator.
It is the intent of the Winslow Family Y-DNA Project Coordinator to ensure to the best of his ability the strict confidentiality of the specific identity of the participant in conjunction with the participant's personal Y-Chromosome DNA profile.

give permission to Stephen J. Winslow, Sr., Winslow Family Y-DNA Project Coordinator, to receive my DNA profile results and affiliated genealogical information.
It is also my understanding that the composite individual data from all participants in the Winslow Family Y-DNA Project will be posted on the Winslow Genealogy web site, as a copyrighted document, and possibly will also appear in other future literary publications, possibly using some or all of the information specified in items 1 thru 7 below. The information in items 5 thru 7 below is utilized to help identify the Most Recent Common Ancestor (MRCA) shared by two or more individual participants.
1. The repeat values recorded for each of the Y-chromosome markers tested in your DNA Profile.
2. The unique number assigned by the Project Coordinator to your DNA Profile results.
3. The Laboratory Control Number specific to your DNA Profile results.
4. The specific spelling of the surname (Winslow, Winflow, etc.) of the participant.
5. The full name of the oldest known progenitor provided in your genealogical information.
6. The date of birth & death of the oldest known progenitor provided in item 5 above.
7. The place of birth & death of the oldest known progenitor provided in item 5 above.
I furthermore assume responsibility for any risk associated with my participation in the Winslow Family Y-DNA Project.
I also agree to indemnify and hold Stephen J. Winslow, Sr. (Project Coordinator) harmless from all claims, judgments and expense incurred in connection with any claims brought as a result of my participation in the Winslow Family Y-DNA Project and which are released by the terms of this agreement.
